What is advanced periodontitis?
Advanced periodontitis occurs in stages III and IV, when the loss of interdental clinical attachment is greater than 5mm and the disease has resulted in the loss of several teeth. In stage IV, other problems may be present, such as masticatory dysfunction, secondary occlusal trauma… or even fewer than 20 teeth remaining in the mouth.
The severity of the pathology may also be related to its grade, i.e. the speed at which it progresses. For example, rapid attachment loss of over 2mm for 5 years also corresponds to advanced periodontitis, the severity of which needs to be taken seriously. This generally corresponds to a faster-than-expected progression in relation to the level of tartar. In some cases, co-morbidity may include heavy smoking or severe diabetes.
How do you treat severe periodontitis?
If you’re showing signs of severe periodontitis, it’s essential to see a periodontal dentist, even if you’ve already lost several teeth or several of them are moving.
Stage III, IV or Grade C periodontitis does not mean that tooth loss is inevitable. Periodontal treatment, consisting of local care and the appropriate number of lithotripsy sessions, and possibly supplemented by medication, can strengthen the teeth’s hold in the jaw and help them stay there.
Lithotripsy is a gentle sonic technique that uses sound vibrations to remove tartar from under the gums, in the same way as for kidney stones. Unlike scaling (which only removes surface tartar), lithotripsy also cleanses and heals periodontal pockets deep down. With regular follow-up, the situation is kept under control.
